Please complete all three to bring with you for your first appointment along with your medical insurance card.
FOR ALL APPOINTMENTS
In order to better serve all of our patients, we do ask that you call and schedule an appointment when you need to be seen by a physician. We will do everything possible to give you a time which is convenient to your schedule and promptly addresses all of your medical needs. If you are unable to keep your appointment or are going to be late, please notify the office as soon as possible.
As a courtesy to patients being seen in the office, your calls regarding medical matters may be taken by the office personnel. After discussing the issue with one of the physicians, one of our staff may return your call. The physicians attempt to return calls from patients who wish to speak directly to them. The majority of these calls are returned after 5:00 pm.
Prescription refill requests should be called into the office as early as possible, but no later than 3:00 pm. Please be aware that it may be necessary for a physician to examine you before the prescription can be approved. Please allow ample time to accomplish this task. No prescription refills will be approved on the weekend. No antibiotics will be called in unless you are seen by a physician.
In general, payment is required at the time of service, unless previous arrangements have been made with the office. We accept payments by cash, check, Mastercard, and Visa. We do not accept insurance assignment except for PPO, HMO, and Medicare patients. All charges are the responsibility of the patients or their guardian. We will provide you with a statement that contains all the information your insurance company should need about your visit.
We must emphasize that our relationship is with you and not with your insurance company. Unless you are covered by an HMO, PPO, or Medicare, your coverage is between you, your employer and your insurance company.
AVAILABILITY OF PHYSICIANS
If calling after hours, or when the physician is not in the office, you may be served by another medical provider. At least one of the physicians is always available for emergency calls. All physicians are affiliated with Lake Pointe Medical Center. If you have an emergency, go directly to the emergency room at Lake Pointe Medical Center.
